The Community Health Nurse-RN (CHN-RN) uses professional knowledge and discretionary judgment to provide assessment, health care planning, and management and ongoing monitoring of individuals who experience developmental disabilities, including adults and children with complex medical conditions and others who also experience mental health diagnoses. The CHN-RN provides consultation and education to individuals, families and staff. He/she must work independently in both urban and very remote communities, and may be called on to respond to crisis situations. Other job duties may include attending medical appointments with individuals and providing training to direct care staff regarding medication administration and assisting with other health needs. The Community Health Nurse has the experience and knowledge to function independently and provide direction to staff and education to families and staff.
