Overview

The Community Support Supervisor - Exempt (CSS-Exempt) provides services to Hope support recipients through their work in teaching, coaching and mentoring direct support staff in order to ensure the delivery of quality services to Hope support recipients. The CSS-Exempt is responsible for assigned Agency Assisted Living Homes and the support recipients who live in them. This position oversees the delivery of services as delineated in the Plans of Care and/or support plans. This includes ensuring the services address the health and safety needs of the support recipients and provide positive learning and inclusion opportunities.

Responsibilities

  1. Effectively manages assigned Agency Operated Assisted Living Homes.
  2. Develops staff through teaching, coaching, and mentoring in the delivery of quality services provided in or based out of Agency Operated Agency Assisted Living Homes. Focuses on developing the leadership and administrative skills of the Home Alliance Coordinators (HAC) and Leads.
  3. Supervises, evaluates, counsels, and disciplines staff, working through the HAC’s and Leads when appropriate. Collaborates in all aspects of onboarding personnel to fulfill service requirements.
  4. Monitors and ensures staff training including home specific and Human Resources requirements compliance. Schedules staff for upcoming training offerings as needed.
  5. Schedules staff in alignment with services delineated in the Plan of Care and/or support plans and in alignment with service delivery requirements.
  6. Participates in the team process, including developing goals and objectives and providing progress information, for the support recipient’s Plan of Care and/or treatment plan.
  7. Monitors service utilization and goal progress for support recipients and implements corrections as necessary. Encourages the health and socialization of support recipients through maximizing use of community resources and services.
  8. Ensures all services are provided in alignment with relevant local, state, and Federal regulations and complies with mandatory reporting responsibilities.
  9. Monitors, reviews, and ensures accurate and timely submission of service documentation from direct support staff to support accurate billing processes. Responds timely to follow-up requests as required.
  10. Coordinates with the Adult Assisted Living Home Licensing Manager and the Deputy Director of the Physical Plant and Safety for initial licensing of Agency Operated Assisted Living Homes, ongoing compliance with licensing regulations, and health and safety standards. Completes annual paperwork for state and Agency requirements as required.
  11. Conducts home visits for all environments as required. Ensures vehicles and homes are in good condition, and checklists and maintenance schedules are followed. Monitors and ensures adequate and appropriate home management including upkeep to Agency and community standards.
  12. Assists Network Director in achieving Agency fiscal objectives buy effectively utilizing and allocating resources and monitoring service utilization.


     

Qualifications

Education and Experience

  1. Bachelor's Degree in Health Science, Social Science, Special Education or related field required. Emphasis of study in developmental disabilities strongly preferred.
  2. Four (4) years' work experience in human services field required, two (2) of which must be held in a supervisory capacity. Case management, service coordination and residential services strongly preferred.
