  • Overview

    The Mental Health Case Manager is responsible for the provision of case management and community based services to individuals on their caseloads, their families, and/or therapeutic home providers.  The Case Manager works with the individual’s team to develop treatment goals and objectives.  Duties include crisis intervention, advocacy, assessment, admission and discharge planning, interdisciplinary teamwork, and acting as a community liaison.  The Mental Health Case Manager accomplishes positive program outcomes through the facilitation of ongoing collaborative and interactive processes between the individual and his/her family/guardian and professional service/treatment providers and/or the natural supports within their community. 


    1. Assures that each individual has ongoing active support in communicating and meeting with family members and guardians, and that there is annual Treatment Plan development and twice yearly reviews.
    2. Assures (through monthly home visits) that all individuals live in a clean, comfortable environment that is safe, has appropriate and nutritious food, and availability of appropriate clothing.
    3. Assures that each individual has been provided opportunities for mental health, medical, dental, dietary and/or health assessments and follow up.
    4. Assures appropriate attendance, facilitation, and preparation (develops agenda) for all Treatment Team and Support Team meetings according to agency standards.
    5. Maintains a current understanding of DBH regulations, COA standards and Hope’s Standard Operating Procedures.
    6. Assures all service notes are completed accurately and within agency timeframes.



    • Bachelor degree in Health Science, Social Science or Special Education with an emphasis of study in developmental disabilities/mental health
    • Two years of experience in mental health, one of which involves responsibility in case management. Must demonstrate knowledge of placement, assessments and evaluations.
    • Equivalent combination of education, training and/or experience



    • Ability to perform job duties with an emphasis on accident prevention and ability to handle emergency situations in a calm and professional manner.
    • Skilled in counseling and crisis intervention.  Demonstrated ability in group facilitation.
    • Understanding of charting and billing to Medicaid standards.
    • Ability to demonstrate understanding of concepts of generalization and normalization, least restrictive environments, interdisciplinary team, and behavior modification, and the tools used in providing these supports.



    • Ability to lift 35 lbs, or 1/3 of body weight, whichever is less, from a squat-and-kneel position
    • Possess a valid driver license & at least 3 years of driving experience (may include permit time)
    • Provide driving record that meets agency guidelines
    • Pass a criminal background check under state regulation
