    Overview

    The Community Health Nurse – Home Based, has direct responsibility for and supervises the health care management of individuals who experience a disability.  The Nurse coordinates activities with other staff members to promote optimum health and wellness for these individuals.

     

    This position will be working in an assisted living home and will provide consultation and education to individuals, families and staff.  Other job duties may include attending medical appointments with support recipients and providing training to direct care staff regarding medication administration and assisting with other health needs. The successful candidate will be able to work independently and may be called on to respond to crisis calls and situations.

     

    Responsibilities

    1. Directs and supervises the performance of health care services to assure proper and consistent care, executes physician’s orders and issues medications.
    2. Participates in the development and implementation of Individual Program Plans designed to implement and monitor specific developmental training and daily care programs for each individual.
    3. Oversees health and medical care of individuals provided by other staff and monitors by observation and review of records to determine if tasks are properly performed. Assures the implementation and documentation of all physician’s orders and health related ISP goals.
    4. Monitors to assure that individuals are properly positioned and that adaptive equipment is properly used.
    5. Issues instructions for health and medical care of individuals to other staff; and monitors, by observation and review of records, to determine if tasks are properly performed.
    6. Assists in orientation and training of other staff members related to health care skills.
    7. Promotes and maintains Interdisciplinary Team contacts and coordination regarding individual’s health, safety and position.
    8. Acts as shift leader on night shift, and co-shift leader with Lead ISS.
    9. Performs treatment as instructed, including suctioning, oxygen therapy, tracheotomy care, administration of medications, and GT/NG tube feedings.

    Qualifications

    Required

    • Must have a valid and current Alaska State Nursing License
    • Ability to demonstrate a comprehensive knowledge of nursing practices, including charting and record keeping, medications and medical terminology
    • ability to handle medically fragile individuals in a calm and professional manner when confronted with emergency situations

    Desired

    • Must have one (1) year of nursing experience supporting individuals who experience developmental disabilities is preferred
    • Mental Health and/or Community Health experience

    Other

    • Pass a criminal background check under state regulation
    • Ability to lift 35 lbs. or 1/3 of body weight, whichever is less, from squat-and -kneel position
    • Possess a valid driver license and at least 3 years of driving experience (may include permit time)
    • Provide driving record that meets agency guidelines
