    Job Summary:

     

    The Hope Studios Support Liaison provides services to Hope support recipients through their work in providing case management, assessment, planning, coordination, evaluation, and advocacy for options and services available in Hope Studios. The Hope Studios Support Liaison serves as a member of the multi-disciplinary team to develop goals and objectives as outlined in each recipient’s Plan of Care, Individualized Educational Plans, and/or other support plans, utilizing strategies in alignment with Hope’s organizational culture and service philosophy. This includes ensuring the services address health and safety needs of support recipients and provides positive learning, inclusion and engagement opportunities by utilizing knowledge of the programs that Hope provides through the Discovery Center. The Hope Studios Support Liaison maintains loyalty to the organization, its beliefs, values, mission, vision, and expectations and to the recipients of services.

    Job Duties:

     

    Essential Job Duties

     

    •  Provides overall coordination and communication of needs and services for each support recipient on their caseload, including interacting with family members/guardians, community-based service providers, and Agency staff.
    •  Completes specified tasks associated with the development of support recipients’ Plans of Care, Individualized Educational Plans, and/or other support plans in accordance with state and federal requirements as it pertains to studio goals and objectives.
    •  Ensures services are designed for continuity of supports between home, school and Hope in areas of socialization, education, time, group dynamics, and community engagement activities.
    •  Develops goals and objectives in partnership with studio staff, support recipients, and their family members/guardians, and in alignment with their desired outcomes.
    •  Maintains knowledge of support recipients’ Plan of Care, Individualized Educational Plan, and/or other support plans to monitor service utilization and goal progress for support recipients. Implements corrections as necessary.
    •  Develops and maintains tracking of Plan of Care and/or support plan dates, relevant due dates, service utilization, materials fee payments, and participants’ schedules. Ensures corrective action plans are developed and implemented as required.
    •  Collaborates effectively and efficiently with other support staff in the participant’s life to ensure comprehensive and quality care is provided to all support recipients.
    •  Acts as a liaison between homes, families, guardians, internal staff, and other service providers to ensure that requested schedules of participation are adhered to and followed as agreed.
    •  Ensures that the Plan of Care, Individualized Educational Plan, and/or support plans, updated goal sheets, notes, and any other related documents are completed and in the recipient’s master file.
    •  Maintains a current understanding of state and federal regulations pertaining to Home and Community Based Waiver services, accreditation standards, and Hope’s Standard Operating Procedures.

    Qualifications

    Job Requirements:

     

    Education and Experience

     

    1. Bachelor’s degree in Health Science, Social Science, Special Education or related field required. Experience in lieu of education may be accepted on a two for one basis in appropriate fields.

    2. Two years of experience working in social services, one year of which involves responsibility for case management or service coordination required.

     

    Additional Requirements

     

    •  Possesses a valid Alaska driver’s license and has at least three years of driving experience (may include permit time). Obtains a driving record at time of employment, annually thereafter, and as requested by management.
    •  Ability to safely operate Agency vehicles when needed. Possesses reliable transportation and vehicle insurance when transporting support recipients or conducting Agency business.
    •  Meets State and Agency requirements for passing a criminal background check upon hire and as required thereafter.
