    Job Summary:


    The Child & Family Support Liaison provides services to Hope support recipients through their work in providing case management, assessment, planning, coordination, evaluation, and advocacy for options and services available to support recipients in the Child and Family Supports network. This position provides services that meet support recipients’ comprehensive health needs through communication of available Agency and community resources, which ensure quality services to the recipient and their families. The Child & Family Support Liaison serves as a member of the multi-disciplinary team to develop goals and objectives as outlined in each recipient’s Plan of Care, Individualized Educational Plans, and/or other support plans, utilizing strategies in alignment with Hope’s organizational culture and service philosophy. This includes ensuring the services outlined address the health and safety needs of the support recipients and provide positive learning, inclusion and engagement opportunities within community-based settings. The Child & Family Support Liaison maintains loyalty to the organization, its beliefs, values, mission, vision, and expectations and to the recipients of services.


    Job Duties:


    Essential Job Duties


    •  Provides overall coordination and communication of needs and services for each support recipient on their caseload, which includes interactions with family members/guardians, community-based service providers, and Agency staff.
    •  Completes specified tasks associated with the development of a support recipient’s Plan of Care, Individualized Educational Plan, and/or other support plans in accordance with state and federal requirements.
    •  Ensures services are designed for continuity of supports between home, school and Hope in areas of socialization, education, time, group dynamics, and community engagement activities.
    •  Develops goals and objectives in partnership with support recipients and their families and in alignment with their desired outcomes.
    •  Knows support recipients’ Plan of Care, Individualized Educational Plan, and/or other support plans. Monitors service utilization and goal progress for support recipients and implements corrections as necessary.
    •  Collaborates effectively and efficiently with other support staff in the Child & Family Supports network to ensure comprehensive and quality care is provided to all support recipients.
    •  Ensures that the Plan of Care, Individualized Educational Plan, and/or support plans, notes, and any other related documents are completed and in the recipient’s master file.
    •  Maintains a current understanding of state and federal regulations pertaining to Home and Community Based Waiver services, accreditation standards, and Hope’s Standard Operating Procedures.


    Job Requirements:


    Education and Experience

    •  Bachelor’s degree in Health Science, Social Science, Special Education or related field required. Experience in lieu of education may be accepted on a two for one basis in appropriate fields.
    •  Two years of experience working in social services, one year of which involves responsibility for case management or service coordination required. Experience working with families and children who experience disabilities strongly preferred.


    Additional Requirements

    •  Possesses a valid Alaska driver’s license and has at least three years of driving experience (may include permit time). Obtains a driving record at time of employment, annually thereafter, and as requested by management.
    •  Able to safely operate Agency vehicles when needed. Possesses reliable transportation and vehicle insurance when transporting support recipients or conducting Agency business.
    •  Meets State and Agency requirements for passing a criminal background check upon hire and as required thereafter.
    •  Lifts 25 pounds from a squat and kneel position on the floor.
